The Effect of Glare on Maddox Rod Test for Prism Correction of Lateral Heterophoria at Distance

Abstract
|
|
|
Purpose: This study was intended to investigate the effect of glare on maddox rod test for prism correction of lateral heterophoria at distance.
Method: We measured lateral heterophoria at distance by maddox rod test under glare and non-glare condition in 60 normal subjects who were selected on the basis of normal binocular function(accommodation, vergence). Maddox rod test was performed with different light sources under bright- and dark illumination in order to induce glare. Also, we measured prism correction of lateral heterophoria at distance by Maddox rod test and modified thorington test under bright illumination with non-glare induced light source.
Results: There were statistically significant difference respectively in exophoria and esophoria in prism correction of lateral heterophoria at distance by Maddox rod test between under bright illumination(800lux) with non-glare induced light source(850cd/m2) and dark illumination(20lux) with glare induced light source(850cd/m2) and previous equivalent dark illumination(20lux) with non-glare induced light source(20cd/m2)[(F=12.16, p=0.00),(F=8.03, p=0.00)], both of them showed tendency toward to esophoria. When non-glare induced light source(850cd/m2) was used by under bright illumination(800lux) in exophoria and esophoria, prism correction of lateral heterophoria at distance by Maddox rod test and modified thorington test were ?2.07¡¾0.43¡â, -2.11¡¾0.67¡â and 1.24¡¾0.31¡â, 1.10¡¾0.36¡â respectively, and there were no statistically significant difference in exophoria and esophoria respectively[(t=0.19, p=0.85), (t=1.32, p=0.20)].
Conclusion: When Maddox rod test is performed under relatively darker space compared with brightness of light source, it may cause glare. Consequently, it is presumed that glare causes near accommodation in lateral heterophoria measurements at distance by Maddox rod test, so that it leads to tendency toward to esophoria and also when maddox rod test is performed at bright space under similar daily illumination with non-glare induced light source, it is relatively easier to control of accommodation.
